Summary: Emergency Departments (EDs) seldom conduct Advanced Care Planning (ACP) during patient visits, possibly due to the difficulty in locating ACP documents in the electronic health record (EHR). A study found that ACP documents are lacking for ED patients, even under optimal conditions with social work availability. The multiple potential locations of ACP documents and inaccurate linkage to the EHR are identified as barriers to ACP conversations.

Comorbidity and the association with 1-year mortality in hip fracture patients: can the ASA score and the Charlson Comorbidity Index be used interchangeably?

Summary: The study found that both the American Society of Anaesthesiologists score (ASA score) and the Charlson Comorbidity Index (CCI) show a stepwise association with 1-year mortality in hip fracture patients, despite measuring different factors and capturing different individuals at risk. Since the ASA score is already accessible for health care staff, it might be preferable to aid in prioritizing vulnerable hip fracture patients at risk of adverse outcomes.
