Locating Advance Care Planning Documents in the Electronic Health Record during Emergency Care

Olivia Pyles et al.

Summary: Emergency Departments (EDs) seldom conduct Advanced Care Planning (ACP) during patient visits, possibly due to the difficulty in locating ACP documents in the electronic health record (EHR). A study found that ACP documents are lacking for ED patients, even under optimal conditions with social work availability. The multiple potential locations of ACP documents and inaccurate linkage to the EHR are identified as barriers to ACP conversations.

Comorbidity and the association with 1-year mortality in hip fracture patients: can the ASA score and the Charlson Comorbidity Index be used interchangeably?

Stina Ek et al.

Summary: The study found that both the American Society of Anaesthesiologists score (ASA score) and the Charlson Comorbidity Index (CCI) show a stepwise association with 1-year mortality in hip fracture patients, despite measuring different factors and capturing different individuals at risk. Since the ASA score is already accessible for health care staff, it might be preferable to aid in prioritizing vulnerable hip fracture patients at risk of adverse outcomes.

Deconstructing the Complexities of Advance Care Planning Outcomes: What Do We Know and Where Do We Go? A Scoping Review

Ryan D. McMahan et al.

Summary: The study found that although there were differences among different ACP interventions, overall, these interventions and outcomes were positive, especially in terms of Process and Action outcomes. Additionally, positive results were also achieved in aspects such as increased satisfaction with communication and care among patients/surrogates and decreased distress among surrogates/clinicians.

Ninety-Day Readmission Rates in a Geriatric Hip Fracture Population, From a Bundled Care Payment Initiative Perspective

Jeffrey M. Bair et al.

Summary: The study found a 90-day readmission rate of nearly 20% in geriatric fragility fracture patients. The application of BCPI definitions linked more readmissions to the index surgical event than previously thought. Further investigation into the risk factors for readmission in this population is needed in light of bundled payment models.

30-day mortality in patients after hip fracture surgery: A comparison of the Charlson Comorbidity Index score and ASA score used in two prediction models

Kristin Haugan et al.

Summary: The study compared two prediction models for 30-day mortality after hip fracture surgery, and found that the Charlson Comorbidity Index (CCI) model and the American Society of Anesthesiologists (ASA) model had equal predictive ability. However, the ASA score may be the preferred tool for predicting 30-day mortality after hip fracture.

The PREPARE for Your Care program increases advance care planning engagement among diverse older adults with cancer

Sarah S. Nouri et al.

Summary: The study found that among older adults, using an interactive multimedia web-based ACP program along with an easy-to-read advance directive can increase ACP documentation and engagement, regardless of cancer diagnosis. This approach did not lead to increased depression or anxiety.

The influence of POLST on treatment intensity at the end of life: A systematic review

Kelly C. Vranas et al.

Summary: This study systematically reviewed the influence of POLST on treatment intensity among patients with serious illness and/or frailty. The findings suggest a moderate strength of evidence that treatment limitations on POLST may reduce treatment intensity, but the evidence base is limited and demonstrates potential unintended consequences.

Improving Patient Preparedness and Confidence in Discussing Advance Directives for End-of-Life Care with Health Care Providers in the United States and Japan

John M. Friend et al.

Summary: The study compared cultural values and attitudes of late-middle-age Japanese and US individuals to identify strategies for increasing advance directive (AD) planning. Important similarities were found in both groups, with individuals valuing self-reliance or interpersonal relationships showing greater preparedness for AD planning. Decision preparedness and positive death attitude predicted greater confidence in discussing care options with a provider in both countries.
