Increasing operator experience and transitioning to retrograde approach when antegrade technique fails have improved success rates in CTO PCI. The upfront retrograde approach is used in some cases with moderate success, and the presence of usable collateral is the most important predictor. While pursuing improved revascularization rates, patient safety and complications rates should also be considered.
Keypoints Increasing operator experience and an algorithmic approach of transitioning to retrograde approach when antegrade technique fails, have improved the success rates in CTO PCI. Upfront (Primary)retrograde approach is used in some cases with moderate success and the presence of usable collateral is the most important predictor of this. While pursuing improved revascularization rates of CTO PCI, one should also keep in mind patient safety and complications rates.
