Summary: The ILUMIEN III trial compared OCT-guided PCI to IVUS or angiography-guided PCI in 450 patients with non-complex lesions, finding no significant differences in 12-month clinical outcomes. No independent predictors for 12-month stent-related clinical events were identified from final OCT analysis. Further research in a larger sample size focusing on complex patients and lesions is needed to confirm the potential clinical benefit of OCT-guided PCI.

Summary: This study aimed to compare the difference in target vessel failure (TVF) at 3 years after intravascular ultrasound (IVUS) guidance versus angiographic guidance for second-generation drug-eluting stent (DES) implantation in all comers. The results showed that IVUS-guided DES implantation significantly reduced the rates of TVF and stent thrombosis (ST) at 3 years, particularly when the IVUS-defined optimal procedure was followed.
