Intensive care unit observation after pancreatectomy: Treating the patient or the surgeon?

Summary: Background: Routine ICU admission following pancreatectomy is common, but its value in avoiding failure-to-rescue is poorly studied. Methods: ICU courses of patients undergoing pancreatectomy from 2013 to 2020 were analyzed. Results: About 63% of patients underwent pancreaticoduodenectomy and had ICU admission. Among them, about 5.3% required ICU-level interventions during ICU admission. Patients requiring immediate ICU-level interventions had significantly higher mortality rates, longer hospital stays, and higher total ICU costs. Conclusion: Patients without immediate postoperative ICU-level interventions have a low risk of failure-to-rescue. Ward admission with a low threshold for care escalation can save costs and unburden ICUs.
