Summary: Severe allograft dysfunction following liver transplantation is a major complication, which can lead to immediate retransplant or death. While the incidence of primary nonfunction (PNF) has decreased, its impact on patients, healthcare teams, and the organ pool remains significant. The diagnosis of PNF lacks a universal test, and criteria for urgent retransplantation vary across healthcare systems. However, a concerted effort to understand PNF, optimize graft recipient selection, improve the clinical environment, and enhance postoperative management has led to a changing impact of traditional risk factors. Machine perfusion strategies have shown promise in reducing the incidence of PNF but cannot completely eliminate the risk. Early identification of severe allograft dysfunction and aggressive management are crucial for salvaging grafts and maintaining optimal physiological state in patients considering retransplantation as the ultimate strategy for salvage.
