Patterns of Alcohol Use After Early Liver Transplantation for Alcoholic Hepatitis

Brian P. Lee et al.

Summary: Early liver transplantation for alcoholic hepatitis can save lives, but concerns about continued alcohol use exist. This study identified distinct patterns of alcohol use post-transplantation and found that early alcohol use was associated with higher risk of mortality at 3 and 5 years. Pre-transplant characteristics were associated with post-transplant alcohol use patterns, which can inform candidate selection and addiction care after transplantation.

Psychosocial Assessment Rather Than Severity of Liver Failure Dominates Selection for Liver Transplantation in Patients With Alcohol-Related Liver Disease

Kimberly E. Daniel et al.

Summary: The study found that patients with alcohol-related liver disease undergoing liver transplantation evaluation have higher vulnerability in psychosocial aspects, with only those with low SIPAT scores being selected. Psychosocial assessment has a significant impact on the selection of ALD patients for liver transplant listing, while having a minor influence on non-ALD candidates.

Recovery and outcomes of patients denied early liver transplantation for severe alcohol-associated hepatitis

Jessica Musto et al.

Summary: This study found that spontaneous recovery in patients with severe AH undergoing LT evaluation is associated with factors such as age, index INR, and lower peak MELD. Most recovered patients may still experience end-stage complications. LT is the only factor associated with lower mortality.

Liver transplantation for hepatocellular carcinoma: impact of expansion criteria in a multicenter cohort study from a high waitlist mortality region

Federico Pinero et al.

Summary: This study compared liver transplantation outcomes for hepatocellular carcinoma under different allocation policies, finding that some policies may help reduce recurrence rates or increase the number of HCC patients eligible for transplantation. However, overly strict policies could raise ethical concerns regarding organ allocation.

Impact of COVID-19 Pandemic on Liver Transplantation and Alcohol-Associated Liver Disease in the USA

Summary: Since the start of the COVID-19 pandemic, ALD has become the most common indication for listing and the fastest increasing cause for LT. Collective efforts are urgently needed to stem the rising tide of ALD on health care resources.
