Evidence of beta amyloid independent small vessel disease in familial Alzheimer's disease

Summary: We studied the pathology of small vessel disease (SVD) in subjects with Familial Alzheimer's disease (FAD) carrying the presenilin 1 (PSEN1) p.Glu280Ala mutation and compared it to those with sporadic Alzheimer's disease (SAD) as well as Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy (CADASIL) with different NOTCH3 mutations. The study found that although FAD subjects did not show significant radiologic evidence of SVD, post-mortem studies revealed similar SVD pathology in cortical areas and basal ganglia in both PSEN1 FAD and CADASIL subjects, with higher rates of arteriosclerosis in CADASIL subjects. Furthermore, the presence of covert SVD was observed in PSEN1 FAD and may contribute to disease progression.
