Insomnia and Insomnia Symptoms in Persons With Heart Failure An Integrative Review

Rida Gharzeddine et al.

Summary: Insomnia and insomnia symptoms in persons with heart failure are associated with various sociodemographic, clinical, and cognitive-behavioral factors, and are linked to outcomes such as depression, fatigue, poor physical functioning, and decreased exercise capacity. However, more research, including larger and longitudinal studies, is needed to further understand the complex relationships between insomnia and potential predictors and correlates in heart failure.

Relationships between Obstructive Sleep Apnea Syndrome and cardiovascular risk in a naive population of southern Italy

Pierluigi Carratu et al.

Summary: This study evaluated cardiovascular risk in 185 OSA patients, finding a strong association between cardiovascular risk and obesity, but a weak relationship with AHI values. Obese individuals had higher scores in the Epworth Questionnaire and Charlson Co-morbidity Index compared to other sub-groups. Further studies are needed to explore the role of inflammation from obesity in increasing cardiovascular risk in OSA patients.

Endothelin-1 and LOX-1 as Markers of Endothelial Dysfunction in Obstructive Sleep Apnea Patients

Monika Kosacka et al.

Summary: The study found that OSA patients had increased serum levels of ET-1 but not LOX-1. ET-1 negatively correlated with LDL levels, while LOX-1 positively correlated with fasting glucose levels and was higher in diabetic patients. In mild OSA patients, LOX-1 was negatively correlated with mean arterial oxygen saturation during sleep, while in severe OSA patients, LOX-1 levels were positively correlated with uric acid levels.

Effects of sleep deprivation on endothelial function in adult humans: a systematic review

Brady J. Holmer et al.

Summary: Sleep deprivation is associated with increased cardiovascular disease (CVD) morbidity and mortality, with age-related alterations potentially exacerbating CVD susceptibility in older individuals. Endothelial dysfunction may play a central role in linking sleep deprivation to CVD, although the exact mechanisms are not fully understood.

Altered regional cerebral blood flow in obstructive sleep apnea is associated with sleep fragmentation and oxygen desaturation

Lirong Yan et al.

Summary: Altered cerebral perfusion patterns were observed in OSA patients compared to healthy reference subjects. This impaired perfusion, mainly affecting the parietal and prefrontal cortices, may contribute to neurocognitive deficits in OSA. Correlation analysis showed distinctive associations between regional hypoperfusion and parameters of intermittent hypoxia and sleep fragmentation, suggesting a potential pathomechanism for neurocognitive deficits in OSA.

OBESITY CARDIOMYOPATHY: EVIDENCE, MECHANISMS, AND THERAPEUTIC IMPLICATIONS

Summary: The rise in heart failure prevalence is partly due to increasing rates of overweight and obesity. Evidence supports the existence of obesity cardiomyopathy as a unique disease entity that develops independently of other heart diseases. Clinical and experimental research have identified pathological changes in myocardial structure and function in obesity, highlighting various cellular and molecular mechanisms underlying this condition.

Summary: Obstructive sleep apnea is common among middle-aged and older adults and often underrecognized, especially in patients with heart disease. Obstructive Sleep Apnea in Adults: What Primary Care Physicians Need to Know

Enrique Arredondo et al.

Summary: Obstructive sleep apnea (OSA) is a prominent disease characterized by recurrent collapse of the upper airway during sleep and is estimated to affect 936 million adults globally. It increases the risk of cardiovascular diseases, metabolic disorders, and decreased quality of life. Current treatment options include lifestyle changes, behavioral modification, mandibular advancement devices, surgical treatment, and continuous positive airway pressure, with the latter being the gold standard.

Summary: Sleep-disordered breathing, including obstructive sleep apnea, is associated with a higher risk of heart failure, cardiovascular disease, and arrhythmias. Despite its significant impact on cardiovascular health, OSA is often underdiagnosed. Clinical evidence shows OSA as an independent risk factor for arrhythmias.
