The management of post-transplantation recurrence of hepatocellular carcinoma

Luckshi Rajendran et al.

Summary: The annual incidence of hepatocellular carcinoma (HCC) is increasing, and liver transplantation is the preferred treatment. However, there is currently no consensus on post-transplantation cancer surveillance and recurrence management for HCC. Early tumor detection, re-evaluation of immunosuppression regimens, and staging are important for guiding decision-making and maximizing treatment effect. Treatment recommendations specific to recurrence type are provided.

Predicting survival after liver transplantation in patients with hepatocellular carcinoma using the LiTES-HCC score

David Goldberg et al.

Summary: A risk score was developed to predict post-transplant survival for liver cancer patients, incorporating both HCC- and non-HCC-related variables. This score showed potential in prioritizing patients based on predicted survival, offering meaningful improvements in life-years at the population level.

Predicting microvascular invasion in hepatocellular carcinoma: a deep learning model validated across hospitals

Shu-Cheng Liu et al.

Summary: A deep learning-based framework was developed for preoperative prediction of microvascular invasion using CT images, without the need for manual feature extraction. The ResNet-18 model showed superior performance in predicting MVI on CT images scanned at multiple hospitals, with attention heatmaps confirming its focus on relevant imaging features.

Outcomes after liver transplantation in Korea: Incidence and risk factors from Korean transplantation registry

Jong Man Kim et al.

Summary: This study analyzed the post-liver transplantation outcomes in the Korean population and identified death, graft loss, rejection, renal failure, new onset of diabetes, and hepatocellular carcinoma recurrence as common outcomes. The incidence of these outcomes was generally higher in DDLT recipients compared to LDLT recipients. Recipient age, DDLT, ABO incompatible transplant, and HCC beyond Milan criteria pre-transplant were identified as significant risk factors associated with the outcomes.

Liver Transplantation in Patients with Hepatocellular Carcinoma beyond the Milan Criteria: A Comprehensive Review

Pierluigi Toniutto et al.

Summary: This review discusses the current data on pretransplant criteria for liver transplantation in HCC patients exceeding the Milan Criteria, based on tumor morphological and histological characteristics validated in clinical practice. It also evaluates the role of HCC biological markers and tumor response to downstaging procedures as new tools for selecting patients with a tumor burden outside of the Milan Criteria for liver transplantation.

Direct-Acting Antiviral Therapy in Liver Transplant Patients With Hepatocellular Carcinoma and Hepatitis C

Chung Sang Tse et al.

Summary: A retrospective cohort study of 171 HCV-HCC transplant recipients found that pretransplant DAA therapy was not associated with increased risks of HCC recurrence, graft failure, or death. However, pretransplant DAA had significantly lower SVR rates compared to posttransplant DAA therapy.
