Why double ovarian stimulation in an in vitro fertilization cycle is potentially unsafe

Angelo Tocci

Summary: This study found that two antral follicle recruitment waves occur in the ovaries of normal women, supporting the use of double ovarian stimulation in a single cycle to benefit poor responder patients. However, the mechanism and safety of the double stimulation strategy in the same cycle have been poorly addressed in previous studies.

Alberto Vaiarelli et al.

Summary: In advanced maternal age and/or poor ovarian reserve patients, immediately undergoing a second stimulation in the same cycle (DuoStim) to obtain <= 3 blastocysts for PGT-A testing was found to be more effective than the conventional approach, resulting in higher live birth rates.

Extended Injection Intervals of Gonadotropins by Intradermal Administration in IVF Treatment

Chao Chin Hsu et al.

Summary: Intradermal administration of recombinant human FSH (rhFSH) for IVF treatment achieves similar outcomes in terms of the number of mature oocytes and clinical pregnancy rate compared to conventional daily subcutaneous rhFSH administration. With fewer injections and a smaller dose, this method provides a cost-effective and patient-friendly regimen.

Summary: This study demonstrates that intermittent vaginal administration of gonadotropins is effective for women with poor ovarian response (POR) undergoing controlled ovarian hyperstimulation (COH), helping them obtain sufficient oocytes.

Ovarian Folliculogenesis and Uterine Endometrial Receptivity after Intermittent Vaginal Injection of Recombinant Human Follicle-Stimulating Hormone in Infertile Women Receiving In Vitro Fertilization and in Immature Female Rats

Chao-Chin Hsu et al.

Summary: Vaginal administration of rhFSH may offer an alternative treatment for women undergoing IVF, showing no significant differences in oocyte retrieval, uterine thickness, and blood flow compared to abdominal administration. Furthermore, pharmacokinetic studies revealed lower clearance and prolonged elimination half-life of rhFSH in women receiving vaginally, while animal studies demonstrated increased follicles and progesterone levels in rats treated with intermittent vaginal injections.

Resumed ovarian function and pregnancy in early menopausal women by whole dimension subcortical ovarian administration of platelet-rich plasma and gonadotropins

Chao Chin Hsu et al.

Summary: The study showed that whole dimension subcortical ovarian administration of platelet-rich plasma with gonadotropin can temporarily restore ovarian functions in women with early menopause, increasing the probability of pregnancy using autologous oocytes.

Mild versus conventional ovarian stimulation for IVF in poor, normal and hyper-responders: a systematic review and meta-analysis

Adrija Kumar Datta et al.

Summary: This study analyzed randomized controlled trials comparing mild and conventional IVF treatments to evaluate clinical outcomes and cost-effectiveness in different types of responders. The results showed similar live birth rates and cumulative live birth rates between mild and conventional IVF treatments, with moderate quality of evidence. Risk of ovarian hyperstimulation syndrome was significantly lower with mild IVF, and costs were also lower compared to conventional IVF.
