An ultrasound study of the long posterior sacroiliac ligament in healthy volunteers and in patients with noninflammatory sacroiliac joint pain

Summary: This study aimed to investigate the sonoanatomy of the long posterior sacroiliac ligament (LPSL) in healthy volunteers and patients with noninflammatory sacroiliac joint pain (SIP) using ultrasound. The LPSLs in patients with SIP were found to have significant thickening, altered structure, and surrounding edema. Combining altered structure or edema with ligament thickening showed good diagnostic accuracy for identifying LPSL pathology in noninflammatory SIP.

Summary: Subchondral T2 flaring is common in prepubertal children's sacroiliac joints, with sacral-predominance and symmetry. Signal changes decrease as sacral apophyses close, and are more frequent in boys. Iliac signal does not correlate with other locations. Flaring that is asymmetrical, greater in ilium than sacrum, or intense in a teenager with closed apophyses may indicate pathologic bone marrow edema.
