Summary: The study compared the safety and efficacy of drug delivery to the brain using 10 ms tone bursts and rapid short-pulse (RaSP) sequence with transcranial focused ultrasound and microbubble exposure. The RaSP sequence did not show measurable improvements over the B10 sequence in terms of blood-brain barrier permeability enhancement for drug delivery, and may present an increased risk of vascular damage.

Neuronavigation-guided focused ultrasound for transcranial blood-brain barrier opening and immunostimulation in brain tumors

Ko-Ting Chen et al.

Summary: Focused ultrasound (FUS) with microbubbles can transiently open the blood-brain barrier (BBB) to increase therapeutic agent penetration, benefiting recurrent glioblastoma (rGBM) treatment. A pilot trial using a neuronavigation combined with a manually operated frameless FUS system for rGBM patients established safety and feasibility, with dose-dependent BBB-opening effect observed. Additionally, FUS-induced immune modulation was demonstrated to convert the immunosuppressive tumor microenvironment into an immunostimulatory one at a higher but safe dosage.

Summary: This study investigated the local movement of water molecules in the rat brain induced by focused ultrasound-induced BBB disruption. Results showed significant changes in ADC and RD signals at different time points post-BBBD, along with upregulated AQP-4 expression. This provides preliminary evidence on how mechanical forces from FUS alter water dynamics through diffusion tensor imaging measures and AQP4 expression.
