Hematologic complications with age in Shwachman-Diamond syndrome

Elissa Furutani et al.

Summary: The study examined 153 subjects with SDS and found that absolute neutrophil counts and hemoglobin levels increase with age, while platelet counts and marrow cellularity decrease. Marrow cellularity was not correlated with blood counts. 17% of subjects developed myeloid malignancies.

Summary: This study reveals the impact of DNMT3A mutations on hematopoiesis in TBRS, including changes in blood parameters, alterations in DNA methylation landscape, and potential occurrence of malignancies. This has important clinical implications for the treatment of TBRS patients and DNMT3A-associated hematological disorders.

CAMT-MPL: congenital amegakaryocytic thrombocytopenia caused by MPL mutations-heterogeneity of a monogenic disorder - a comprehensive analysis of 56 patients

Manuela Germeshausen et al.

Summary: CAMT-MPL is a rare inherited bone marrow failure syndrome characterized by isolated thrombocytopenia at birth progressing to pancytopenia. Clinical courses vary greatly, with severe complications including intracranial bleedings and aplastic anemia. Approximately one quarter of patients do not exhibit thrombocytopenia at birth, and there is a higher rate of non-hematological abnormalities, especially relating to the head region.

Specifications of the ACMG/AMP variant interpretation guidelines for germline TP53 variants

Cristina Fortuno et al.

Summary: Germline pathogenic variants in TP53 are associated with Li-Fraumeni syndrome, a cancer predisposition disorder inherited in an autosomal dominant pattern. Intense cancer surveillance for individuals with these variants can reduce cancer-related mortality. Accurate classification of germline variants is crucial for ensuring appropriate cancer surveillance recommendations.

Summary: This study provides insights into the genetic determinants of disease progression and clinical outcomes in individuals with myelodysplastic syndromes, particularly those with SAMD9 and SAMD9L mutations. The findings suggest that somatic genetic rescue is common in SAMD9/9L(mut) MDS, showcasing the exceptional plasticity of hematopoiesis in children.

Distinct genetic pathways define pre-malignant versus compensatory clonal hematopoiesis in Shwachman-Diamond syndrome

Alyssa L. Kennedy et al.

Summary: Research on germline genetic leukemia predisposition in Shwachman-Diamond syndrome identified early emergence of multiple hematopoietic clones with EIF6 or TP53 mutations. The germline SBDS deficiency constrains clone selection via compensatory EIF6 inactivation and maladaptive TP53 mutations, with subsequent leukemia linked to biallelic TP53 alterations. These findings offer insights into clinical surveillance strategies.

Natural gene therapy by reverse mosaicism leads to improved hematology in Fanconi anemia patients

Maria Jose Ramirez et al.

Summary: Fanconi anemia is a genetic disorder characterized by chromosome fragility, bone marrow failure, and predisposition to cancer. A study in Spain found that mosaic FA patients had more favorable long-term clinical outcomes, with some showing multilineage mosaicism. This reverse mosaicism was associated with older age, lower percentage of aberrant cells, and more stable hematology in these patients.

Uterine natural killer cells and recurrent spontaneous abortion

Yao-Yao Fu et al.

Summary: RSA, defined as two or more consecutive pregnancy losses, poses a significant challenge for women of childbearing age, with evidence suggesting an immune background to the condition. Uterine natural killer (uNK) cells, as part of the innate immune system, play a crucial role in pregnancy by accounting for around 70% of total lymphocytes. Understanding the phenotype, origin, receptor, and function of uNK cells can shed light on their relationship with RSA.

Christopher R. Reilly et al.

Summary: Research indicates that rare TERT variants are associated with an increased risk of developing MDS and nonrelapse mortality, with 90% of the variants showing impairment in telomere elongation function. Routine screening for TERT rare variants in MDS patients may enhance transplant outcomes.

Hematopoietic stem cell transplantation in children and adolescents with GATA2-related myelodysplastic syndrome

Summary: In pediatric MDS, patients with GATA2 mutations who underwent HSCT had a 5-year overall survival of 75% and disease-free survival of 70%. Non-relapse mortality and relapse were equal contributors to treatment failure. Organ toxicities and infection rates did not show significant increase, suggesting early consideration of HSCT in young individuals with GATA2 deficiency.

Kathryn A. Six et al.

Summary: This study reports the first two cases of B-cell acute lymphoblastic leukemia (B-ALL) in patients with confirmed germline RUNX1 mutations. One patient had a known RUNX1-FPD diagnosis with a specific mutation, while the other had a novel RUNX1 variant. The clinical course, treatment approaches, and outcomes of the patients are discussed, highlighting the importance of testing for germline RUNX1 mutations in B-ALL patients with relevant family or personal history.
