Circumferential sulcus-guided resection technique for improved outcomes of low-grade gliomas

Summary: The study identified NgR1 as a key player in suppressing myelin-associated infiltration in glioblastoma cells through deubiquitinating activity of USP1. Pimozide upregulates NgR1 by post-translational targeting of USP1, sensitizing glioma stem cells to myelin inhibition. Downregulation of NgR1 expression is associated with highly infiltrative characteristics and poor survival in primary human glioblastoma.

Microglia in HGGs play a dual role, with activation in M1 phenotype exhibiting a scavenger-tumoricidal role, while activation in M2 phenotype favoring tumor growth and infiltration. Modulation of microglia-glioma cross-talk may lead to more effective therapies.

Summary: Central tumor location, particularly in the corpus callosum and basal ganglia, was associated with shorter overall survival. Tumors in the left temporal lobe pole also showed reduced overall survival, while tumors in specific regions of the right temporal lobe and white matter were associated with longer survival. Additionally, distance from the center of the third ventricle to the contrast-enhancing tumor border may be a useful prognostic factor, especially in elderly patients.

The standard therapies currently available do not consider the role of glial cells in the tumor microenvironment, which may be one of the reasons for the poor treatment outcomes in patients with high-grade gliomas. The importance of variation in the oxidative phosphorylation rate and regulation in cell metabolism and functional heterogeneity is highlighted, suggesting a basis for multi-targeted, time-dependent therapeutic strategies.
