Summary: This observational study explored the immune response to SARS-CoV-2 vaccination in patients with autoimmune hepatitis (AIH) and patients with cholestatic autoimmune liver disease (primary sclerosing cholangitis [PSC] and primary biliary cholangitis [PBC]). The results showed that AIH patients had a higher rate of SARS-CoV-2 infection and impaired B- and T-cell response to the vaccine compared to PBC and PSC patients, even in the absence of immunosuppression. Monitoring the antibody response to vaccination in AIH patients and considering early booster immunizations for low responders is important.

Outcome of COVID-19 in Patients With Autoimmune Hepatitis: An International Multicenter Study

Cirrhosis was identified as the strongest predictor for severe COVID-19 in AIH patients. Continuing immunosuppression during COVID-19 did not increase the risk for severe COVID-19, but rather lowered the risk for new-onset liver injury during COVID-19.

Acute autoimmune-like hepatitis with atypical anti-mitochondrial antibody after mRNA COVID-19 vaccination: A novel clinical entity?

Summary: This case report describes a 63-year-old man who developed acute severe autoimmune-like hepatitis seven days after receiving the mRNA1273 SARS-CoV-2 vaccine, despite having no history of autoimmunity or natural infection with the virus. The patient displayed unique autoimmune liver serology patterns, including an atypical anti-mitochondrial antibody (AMA) and a distinct type of anti-nuclear antibody (ANA). Treatment with prednisone was effective for this emerging condition.
