Does the Degree of Platelet Adenosine Diphosphate and Arachidonic Acid Receptor Inhibition Correlate With the Severity of Injury in Non-Brain-Injured Trauma Patients?

Grant Woodruff et al.

Summary: This study evaluated the percent inhibition of ADP and AA receptor inhibition in non-TBI patients and correlated it with injury severity and outcomes. The study found that despite a high degree of platelet dysfunction, the only correlation identified was between AA receptor inhibition and hospital length of stay. Further research is needed to determine the clinical significance of thromboelastography with platelet mapping in trauma care.

Impact of platelet transfusion on outcomes in trauma patients

S. R. Hamada et al.

Summary: This study aims to evaluate the impact of early platelet transfusion on 24-hour all-cause mortality and to describe the relationship between platelet count at admission and trauma severity and outcome. The results show that a decrease in platelet count is associated with increased mortality, and early platelet transfusion is associated with a decrease in mortality in patients with severe hemorrhage.

Platelet Dysfunction after Traumatic Brain Injury: A Review

Christina M. Riojas et al.

Summary: Coagulopathy is a known complication of traumatic brain injury (TBI) that can increase morbidity and mortality. Platelet dysfunction is a common occurrence following moderate-severe TBI, and platelet function assays along with platelet transfusion may improve clinical outcomes. However, variability in the literature and retrospective nature of the data present challenges in developing evidence-based management guidelines.

Trauma-induced coagulopathy

Ernest E. Moore et al.

Summary: Trauma-induced coagulopathy describes alterations in hemostasis that can result in both uncontrolled bleeding and hypercoagulation, leading to venous thromboembolism and multiple organ failure.

Pathophysiology of Trauma-Induced Coagulopathy

Patricia Duque et al.

Summary: Trauma-induced coagulopathy (TIC) is an abnormal hemostatic response secondary to trauma, with early and late types defined. Early TIC refers to ineffective hemostasis leading to uncontrolled bleeding in shocked patients, while late TIC is characterized by a hypercoagulable state contributing to thromboembolic events. TIC is not a uniform phenotype, ranging from bleeding to pro-thrombotic profiles, and it is mainly based on alterations in endothelial function, fibrinolysis regulation, and platelet behavior after major trauma.

Epidemiology of Massive Transfusion - A Common Intervention in Need of a Definition

Zoe K. McQuilten et al.

Summary: While massive transfusion (MT) recipients are a small proportion of all transfused patients, they account for about 10% of blood products issued and are mainly used to support major bleeding in surgical, trauma, and gastrointestinal hemorrhage patients. The clinical context, number of blood products required, patient age, and comorbidities are key predictors of short- and long-term survival outcomes. A standard, universally accepted definition of MT is urgently needed to improve care and outcomes for patients with major bleeding.

Mild Head Trauma: Is Antiplatelet Therapy a Risk Factor for Hemorrhagic Complications?

Gabriele Savioli et al.

Summary: Patients receiving antiplatelet therapy who experience mild head trauma do not have a higher risk of bleeding, but they tend to have more severe clinical conditions, including a greater need for surgery and hospitalization.
