Clinical Efficacy and Nephrotoxicity of the Loading Dose Colistin for the Treatment of Carbapenem-Resistant Acinetobacter baumannii in Critically Ill Patients

Wasan Katip et al.

Summary: The aim of this study was to assess the 30-day survival and nephrotoxicity in critically ill patients with carbapenem-resistant Acinetobacter baumannii (CRAB) infection who received non-loading dose (LD) versus LD of colistin methanesulfonate (CMS). The study found that the LD CMS group had higher survival rates, clinical response, and microbiological response compared to the non-LD CMS group. However, LD CMS was associated with an increase in nephrotoxicity, requiring close monitoring of renal function.

Clinical Efficacy and Nephrotoxicity of Colistin Alone versus Colistin Plus Vancomycin in Critically Ill Patients Infected with Carbapenem-Resistant Acinetobacter baumannii: A Propensity Score-Matched Analysis

Wasan Katip et al.

Summary: The study found that there were no significant differences between colistin plus vancomycin and colistin alone in terms of 30-day mortality, clinical response, microbiological response when treating infections due to CRAB. Therefore, combination therapy of colistin with vancomycin is not necessary for managing CRAB infections.

Bloodstream Infections Caused by Klebsiella pneumoniae Carbapenemase-Producing P. aeruginosa Sequence Type 463, Associated With High Mortality Rates in China: A Retrospective Cohort Study

Hangbin Hu et al.

Summary: The study found that KPC-producing CRPA belonging to Sequence Type 463 had a significant impact on bloodstream infections in East China, with a higher mortality rate and resistance to antimicrobials compared to non-ST463 strains. The ST463 CRPA clone showed a high virulence in a larvae model and harbored the bla(KPC) and exoU/exoS genes, indicating a potential new threat in clinical settings. Surveillance of this high-risk clone should be strengthened in China and globally in the future.

Carbapenem resistance in Acinetobacter baumannii, and their importance in hospital-acquired infections: a scientific review

M. Nguyen et al.

Summary: The increasing global prevalence of carbapenem-resistant Acinetobacter baumannii poses a significant threat to public health, highlighting the urgent need for better understanding of this pathogen and the development of novel treatments and control strategies. This review provides an overview of carbapenem, its importance in hospital medicine, and the peculiarities of the bacterial pathogen A. baumannii, including its virulence factors and resistance mechanisms. Various control strategies for combating carbapenem resistance in hospitals and limiting outbreaks are also discussed.

Summary: This study reveals how host immunity and natural selection interact to drive the rapid changes in antibiotic resistance during infection. The development and decrease of resistant strains post-treatment result in dynamic changes in antibiotic resistance during the infection process.

Acinetobacter baumannii: An Ancient Commensal with Weapons of a Pathogen

Meysam Sarshar et al.

Summary: Acinetobacter baumannii is a life-threatening pathogen associated with community-acquired and nosocomial infections, mainly pneumonia. The rise in antibiotic-resistant strains reduces effective therapies and increases mortality. Comparative genomic studies have unraveled the innate and acquired virulence factors of A. baumannii.

Summary: Acinetobacter baumannii is a common nosocomial pathogen with high carbapenem resistance rates in Chinese hospitals. The predominant clone carrying bla(OXA-23) gene is CC208. Multi-drug resistant A. baumannii isolates exhibit various virulence factors related to bacterial motility and polysaccharide biosynthesis.

Summary: The study found that S. Typhimurium was the predominant serotype among Salmonella strains in Nantong city, China, with a high rate of multi-drug resistance. S. Enteritidis and S. Rissen were major serotypes among patients, while S. London, S. Derby, and S. Meleagridis were significant serotypes among asymptomatic people.

Marianna Meschiari et al.

Summary: By reinforcing hand hygiene, extending screening, implementing contact precautions, increasing environmental sampling, and using a cycling radical environmental cleaning and disinfection procedure, the spread of CRAB in the ICU was effectively controlled. Cohorting, admission restrictions, or ICU closures were not necessary.
