Carotid web: an under-recognized and misdiagnosed ischemic stroke etiology

Ehizele M. Osehobo et al.

Summary: The study evaluated the misdiagnosis rates and diagnosis trends of carotid web (CaW), finding that CaW was mainly identified in patients with strokes/TIAs rather than asymptomatic patients, and commonly overlooked in facilities with lower levels of cerebrovascular certification. Recognition of CaW at comprehensive stroke centers (CSC) has significantly increased over time.

Summary: This study compared hemodynamic parameters in the internal carotid artery bulbar segment between patients with Carotid webs (CaWs) and those with atherosclerotic lesions. Results indicated that CaWs result in larger local hemodynamic disruption than mild/moderate carotid atherosclerotic lesions.

Ultrasonic characteristics of carotid webs

Zhifei Ben et al.

Summary: This study retrospectively analyzed ultrasonographic images of 46 cases of carotid web, identifying common characteristics such as location at the initial segment or bifurcation of the carotid artery, presence of thrombi and plaques, and direction of growth. Recognizing these typical features on ultrasound can improve diagnostic accuracy for carotid web.

Carotid webs associated with ischemic stroke. Updated general review and research directions

Summary: Carotid web, a rare intimal variant of fibromuscular dysplasia, is strongly associated with ipsilateral cerebral infarction, predominantly in middle-aged females. It features a shelf-like intraluminal defect on imaging and is believed to cause artery-to-artery embolism mechanism, with potential recurrence prevented by surgical or stenting interventions.

Epidemiological Analysis of 37,424 Carotid Artery Stenosis Intervention Procedures During 11 Years in the Public Health System in Brazil: Stenting has Been More Common Than Endarterectomy

Nelson Wolosker et al.

Summary: In Brazil, the frequency of carotid artery stenting procedures far exceeds that of carotid endarterectomy. While the in-hospital mortality rates for carotid artery stenting are significantly lower than those for carotid endarterectomy, the cost of stenting procedures is significantly higher. This study provides preliminary insights into the fact that the low adoption of carotid endarterectomy in Brazil contrasts with countries where utilization rates are higher for endarterectomy than for stenting.

Justine Sudre et al.

Summary: Although reperfusion therapy was less successful in older patients, these patients may benefit from acute recanalization despite their age. With an increasing older adult population, high-quality prospective studies are still required to better predict functional outcome and clarify the criteria that would allow better selection of appropriate treatment.

Carotid webs in large vessel occlusion stroke: Clinical, radiological and thrombus histopathological findings

Aurora Semerano et al.

Summary: Carotid webs are a rare cause of embolic stroke, particularly in young African women. Endovascular thrombectomy and revascularization procedures have shown to be successful in treating large vessel occlusion stroke related to carotid webs. Histopathological analysis of retrieved thrombi revealed mixed compositions with variable red blood cell content, providing new insights into stroke pathogenesis in patients with symptomatic carotid webs.
