Summary: In patients receiving intestinal transplants, long-term multilineage blood chimerism often develops, especially with donor T cell macrochimerism. These macrochimeras tend to have high GvH to HvG T cell clonal ratios in their allografts, suggesting a potential role in reducing rejection without causing GVHD. The migration of tissue-resident GvH-reactive donor T cells into the recipient circulation and bone marrow may contribute to hematopoietic cell destruction and the engraftment of graft-derived HSPCs to maintain chimerism.
