Strategic Infarct Locations for Poststroke Depressive Symptoms: A Lesion- and Disconnection-Symptom Mapping Study

Summary: This study used lesion-symptom mapping to identify infarct locations and white matter disconnections associated with poststroke depressive symptoms (PSDS). The results showed that infarcts in the right amygdala, right hippocampus, and right pallidum were consistently associated with PSDS. Disconnectome-based analyses revealed disconnections in the right parahippocampal white matter, right thalamus and pallidum, and right anterior thalamic radiation were significantly associated with PSDS.

However, little attention has been given to the issue of measurement noninvariance, which refers to the inability of a measurement instrument to accurately measure the same construct across different individuals or time points. In this article, using the example of inflammatory phenotypes of depression, the authors discuss the implications of differential biology-symptom associations on measurement noninvariance and its impact on the broader diagnostic construct.

A network approach was used to explore the interconnection among depressive symptoms and different measures of somatic disease burden in older adults. Suicidal thoughts, reduced appetite, and cognitive difficulties were identified as consistent bridge connections between depression symptoms and disease clusters, as well as overall somatic disease burden.

While the frontal lobes and limbic system regions are prominent targets for MDD biomarkers, additional areas like the brainstem and midbrain may also play a role. Predicting treatment responses in MDD involves factors like gray matter volumes, white matter tracts, and receptor bindings in specific brain regions, as well as potential targets like the fronto-limbic areas and reward regions for different therapies.

Association of Lesion Pattern and Dysphagia in Acute Intracerebral Hemorrhage

Strategic infarct locations for post-stroke cognitive impairment: a pooled analysis of individual patient data from 12 acute ischaemic stroke cohorts

Summary: This study provides the first comprehensive map of strategic infarct locations associated with risk of PSCI and derived a location impact score that robustly predicted PSCI across cohorts. Through leave-one-cohort-out cross-validation, it was found that the univariable model with only the location impact score had similar performance to the multivariable model with other PSCI predictors, while allowing for easy visual assessment. The development of a quick and reliable visual rating scale may be useful for clinicians in identifying individual patients at risk of PSCI in the future.
