Summary: This experimental study on a swine model of mild acute respiratory distress syndrome found that different levels of positive end-expiratory pressure (PEEP) and external resistances during spontaneous breathing and mechanical ventilation can influence inspiratory intraparenchymal gas redistribution. High PEEP and low external resistances reduced inspiratory pendelluft, while controlled mechanical ventilation, low PEEP, and high external resistances favored larger phenomena of intraparenchymal gas redistribution during the flow-dependent phase of inspiration, potentially endangering lung stability.
