Patients with neutrophilic COPD are susceptible to significant microbiome shifts during exacerbations, especially those in a stable state at baseline. Monitoring the temporal variability of the airway microbiome may provide insight into patient inflammatory status over time.

Summary: The microbiome in bronchiectasis patients is relatively stable over time, with lower diversity associated with disease severity, decreased lung function, and worsened symptoms. Dominance of certain bacteria, such as Pseudomonas, is linked to severe disease, increased inflammation, and more frequent exacerbations. Patients with a Pseudomonas-dominant microbiome have a higher risk of mortality and experience more exacerbations during follow-up compared to those with other dominant genera.
