Genomic Analysis of the Predominant Strains and Antimicrobial Resistance Determinants Within 1479 Neisseria gonorrhoeae Isolates From the US Gonococcal Isolate Surveillance Project in 2018

Jennifer L. Reimche et al.

Summary: Genomic analysis of Neisseria gonorrhoeae isolates from the US in 2018 revealed that most isolates belonged to 33 multilocus sequence types, clustered in 23 major phylogenetic clusters, and had common antimicrobial resistance determinants. This study sets the basis for future research and establishes a genomic baseline for monitoring GC populations.

Emergence and evolution of antimicrobial resistance genes and mutations in Neisseria gonorrhoeae

Koji Yahara et al.

Summary: This study investigated the evolution and spread of drug-resistant Neisseria gonorrhoeae strains in Japan and globally. The emergence of mosaic penA alleles in ST-1901 and ST-7363 was found to be due to recombination events with other Neisseria species, followed by acquisition of wildtype penA alleles. Furthermore, fluoroquinolone resistance mutations in ST-7363 were found to be due to independent mutations rather than horizontal gene transfer. These findings highlight the role of antibiotic use and genetic recombination in driving the spread of drug-resistant gonorrhea.

Choosing New Therapies for Gonorrhoea: We Need to Consider the Impact on the Pan-Neisseria Genome. A Viewpoint

Chris Kenyon et al.

Summary: While new gonorrhoea treatment guidelines typically focus on resistance in Neisseria gonorrhoeae, it is important to consider resistance in commensal Neisseria species as well, as resistance patterns can be passed on to N. gonorrhoeae. This highlights the importance of evaluating the impact of gonococcal therapies on resistance in commensal Neisseria.

Antimicrobial Resistance Profiles of Human Commensal Neisseria Species

Maira Goytia et al.

Summary: The study revealed that commensal Neisseria species may exhibit higher levels of antibiotic resistance compared to pathogenic Neisseria gonorrhoeae, with shared resistance genes. This suggests that commensal Neisseria could potentially serve as a reservoir for the exchange of resistance genes with pathogenic N. gonorrhoeae, impacting drug development and clinical treatment.

Evidence of Horizontal Gene Transfer of 50S Ribosomal Genes rplB, rplD, and rplY in Neisseria gonorrhoeae

Sheeba Santhini Manoharan-Basil et al.

Summary: Horizontal gene transfer has been shown to play a key role in antimicrobial resistance in Neisseria gonorrhoeae, with evidence of HGT found in the genes coding for ribosomal proteins in the Neisseria genus. Recombination events were predicted in N. gonorrhoeae and several commensal Neisseria species, with strong associations between HGT in ribosomal proteins and reduced susceptibility to macrolides. This study is the first to find evidence of HGT-mediated chimerization of ribosomal proteins in bacteria.

Using a public database of Neisseria gonorrhoeae genomes to detect mutations associated with zoliflodacin resistance

Paul C. Adamson et al.

Summary: This study aimed to determine the prevalence of three amino acid mutations associated with zoliflodacin resistance in nearly 13,000 Neisseria gonorrhoeae genomes. The findings suggest that the prevalence of these mutations is very low in N. gonorrhoeae, highlighting the need for further research into mechanisms of zoliflodacin resistance. Genomic epidemiology platforms like PathogenWatch can play a role in enhancing global surveillance of antimicrobial resistance.

Antibiotic Treatment Regimes as a Driver of the Global Population Dynamics of a Major Gonorrhea Lineage

Magnus N. Osnes et al.

Summary: A study on Neisseria gonorrhoeae multilocus sequence type (ST) 1901 found that this lineage is commonly associated with treatment failure, particularly due to alleles linked to reduced susceptibility to extended-spectrum cephalosporins (ESCs). The genetic diversity of ST-1901 was classified into minor and major clades, both originating in East Asia and dispersing globally in waves, with one clade acquiring a gene that significantly reduces susceptibility to ESCs. Despite declining effective population sizes, the lineage has become a reservoir for clones resistant to ceftriaxone, a key drug for gonorrhea treatment.
