The impact of false positive COVID-19 results in an area of low prevalence

Brendan Healy et al.

Summary: While false negative results in COVID-19 testing are well recognized and discussed, false positive results, although less common, also have significant implications, such as potential exposure of non-infected individuals to the virus. These consequences are more significant in high-prevalence settings, despite false positive results being proportionally greater in low prevalence settings. Recommendations to minimize the risks and impacts of false-positive results are provided.

Test sensitivity is secondary to frequency and turnaround time for COVID-19 screening

Daniel B. Larremore et al.

Summary: The COVID-19 pandemic has led to a public health crisis due to the spread of SARS-CoV-2 through presymptomatic, symptomatic, and asymptomatic individuals. Effective population screening for the virus is essential for reopening societies and controlling its spread, with testing frequency and reporting speed playing crucial roles in its success. Test sensitivity is only marginally important compared to these factors.

Remote-Controlled and Pulse Pressure-Guided Fluid Treatment for Adult Patients with Viral Hemorrhagic Fevers

Marcus J. Schultz et al.

Summary: Viral hemorrhagic fevers often lead to circulatory shock, requiring optimized fluid therapy for management. However, there is a lack of data and guidelines in this area. An innovative monitoring approach proposes remote-controlled and pulse pressure-guided fluid resuscitation to enhance supportive care capacity and reduce the risk of virus transmission.

Field evaluation of a rapid antigen test (Panbio™ COVID-19 Ag Rapid Test Device) for COVID-19 diagnosis in primary healthcare centres

Eliseo Albert et al.

Summary: The Panbio (TM) COVID-19 Ag Rapid Test Device performed well as a POC test for early diagnosis of COVID-19 in primary healthcare centres, with a specificity and sensitivity of 100% and 79.6% respectively. The negative predictive value of RAD was 99% and 97.9% under estimated prevalence of 5% and 10% respectively.

Scaling up COVID-19 rapid antigen tests: promises and challenges

Rosanna W. Peeling et al.

Summary: WHO recommends antigen-detection rapid diagnostic tests (Ag-RDTs) with at least 80% sensitivity and 97% specificity for patients with symptoms consistent with COVID-19. Ag-RDTs may show high false negative rates after the acute phase, suggesting the need for a combination of molecular and serological tests. In low prevalence settings and situations where infection needs to be ruled out, Ag-RDTs with high negative predictive values can be used confidently, but high false positive rates in low prevalence areas require further confirmation.

Performance and operational feasibility of antigen and antibody rapid diagnostic tests for COVID-19 in symptomatic and asymptomatic patients in Cameroon: a clinical, prospective, diagnostic accuracy study

Yap Boum et al.

Summary: The study evaluated the performance of rapid diagnostic tests for detecting SARS-CoV-2 infection in the Cameroonian community. It found that rapid diagnostic tests had good overall sensitivity and could be used as an alternative to PCR, reducing diagnostic delays and viral transmission.

The burden of active infection and anti-SARS-CoV-2 IgG antibodies in the general population: Results from a statewide sentinel-based population survey in Karnataka, India

Giridhara R. Babu et al.

Summary: This study estimated the burden of active infection and anti-SARS-CoV-2 IgG antibodies in Karnataka, India, while assessing geographical and risk group variations. Results showed an adjusted total prevalence of COVID-19 at 27.7%, with IgG antibodies at 16.8% and active infection fraction at 12.6%. Rapid antigen detection tests had higher sensitivity in symptomatic individuals compared to asymptomatic individuals.

Multi-site assessment of rapid, point-of-care antigen testing for the diagnosis of SARS-CoV-2 infection in a low-prevalence setting: A validation and implementation study

Stephen Muhi et al.

Summary: The study evaluates the performance and implementation of rapid antigen-based SARS-CoV-2 testing in a setting with low prevalence of COVID-19 infections. Results show high specificity and relatively high sensitivity in early-stage symptomatic patients, while also identifying implementation challenges for consideration in future testing strategies.

Signs and symptoms to determine if a patient presenting in primary care or hospital outpatient settings has COVID-19

Thomas Struyf et al.

Summary: The majority of individual signs and symptoms reviewed show poor diagnostic accuracy, influenced by selection bias and study heterogeneity. Data currently available suggest that neither the absence nor presence of signs or symptoms are accurate enough to confirm or rule out COVID-19. The presence of anosmia or ageusia may serve as a red flag for COVID-19, while fever or cough, due to their high sensitivities, can also be used to target individuals for further testing. Prospective studies evaluating combinations of signs and symptoms in unselected populations are urgently needed to better understand the syndromic presentation of COVID-19.
