Dysregulation of the NLRP3 Inflammasome in Diabetic Retinopathy and Potential Therapeutic Targets

Summary: Diabetic retinopathy is a neurovascular disorder caused by chronic glycemic dysregulation in elderly diabetic patients. It leads to irreversible visual impairment in working age adults due to fluid infiltration in the macula. Current treatments focus on slowing down the disease progression but are not sustainable from an economic and quality of life perspective. Chronic low-grade inflammation in the diabetic eye seems to contribute to the development of diabetic retinopathy. The NLRP3 inflammasome, found in retinal cells, plays a role in the pathogenesis of the disease by upregulating the release of pro-inflammatory cytokines.

Summary: The study found that patients with diabetes and DR had higher levels of IL-6, IL-12, IL-17a, and TNF alpha in their serum compared to healthy controls. TNF alpha showed a higher concentration in PDR group compared to healthy controls, NDR, and NPDR subjects. There was a positive correlation between the presence and severity of DR with clinical parameters and the concentration of serum cytokines IL-6 and TNF alpha.

Summary: Recent research has shown that connexin43 hemichannel blockers can inhibit activation of the inflammasome pathway in diabetic retinopathy. The development of an ex vivo human retinal organotypic culture model provides a more accurate representation of the inflammatory process in diabetic retinopathy, supporting the potential use of connexin43 hemichannel blockers as new therapies for the disease.

However, preventive treatment did not confer visual acuity benefit compared with observation plus treatment with aflibercept only after development of PDR or vision-reducing CI-DME within two years.
