Summary: The study found significant changes in clot formation and lysis in COVID-19 patients, particularly in increased fibrin contribution to maximum clot firmness and prolongation of lysis time. These findings may contribute to our understanding of the hypercoagulable state in critically ill patients with COVID-19.

Recommendations on Inpatient Treatment of Patients With COVID-19

Summary: Whether hospital admission is necessary for COVID-19 patients depends on age, comorbidities, respiratory rate, and oxygen saturation. Adequate oxygenation, pharmaceutical prevention of thrombosis, and administration of dexamethasone are crucial elements in the treatment of severe COVID-19 cases.

Summary: Elevated CRP levels at initial hospital presentation in COVID-19 patients are associated with an increased risk of venous thromboembolism, acute kidney injury, critical illness, and mortality. There is a dose-response relationship between CRP concentration and adverse outcomes, with patients with high levels of both CRP and D-dimer showing the greatest risk of adverse outcomes. Systemic inflammation, as measured by CRP, plays a significant role in predicting clinical outcomes in COVID-19.

Summary: Analysis of 1122 COVID-19 patients revealed that high upper respiratory tract viral load was associated with older age, comorbidities, severe disease (COVID-19), intubation, and mortality, as well as longer stays in the intensive care unit and intubation time. High viral load was also more common in patients with chronic cardiovascular disease, hypertension, chronic pulmonary disease, immunosuppression, obesity, and chronic neurological disease.

Incidence of VTE and Bleeding Among Hospitalized Patients With Coronavirus Disease 2019 A Systematic Review and Meta-analysis

David Jimenez et al.

Summary: The pooled incidence of VTE and bleeding among hospitalized patients with COVID-19 was 17.0% and 7.8% respectively. Factors such as routine screening, critical illness, inclusion of distal DVT and subsegmental PE, and prospective studies were associated with higher rates of VTE. The incidence of bleeding events was sensitive to escalated doses of anticoagulants and the nature of data collection.

Summary: The study found that severe and non-severe hospitalized COVID-19 patients have nearly 6 times and 3 times higher odds of developing symptomatic VTE compared to general medical inpatients. VTE commonly develops at a median of 21 days after symptom onset and independent risk factors include advancing age, cancer, longer interval from symptom onset to admission, lower fibrinogen, and higher D-dimer levels.

Among 223 patients, the survival rate to ICU discharge was 65%, and it was 56% among patients requiring MV. Patients showed a high rate of septic complications during their ICU stay.

Summary: This study compared the effect of intermediate-dose and standard prophylactic anticoagulation on a composite outcome among COVID-19 patients admitted to the ICU. The results showed no significant difference between the two groups in terms of thrombotic events, ECMO therapy, or mortality within 30 days. The study does not recommend routine use of intermediate-dose prophylactic anticoagulation in unselected ICU patients with COVID-19.

SARS-CoV-2 Blood RNA Load Predicts Outcome in Critically Ill COVID-19 Patients

Fabian Heinrich et al.

Summary: The study found that in critically ill COVID-19 patients, blood SARS-CoV-2 RNA load can serve as a prognostic factor, with significantly higher levels in non-survivors upon ICU admission and a higher probability of blood RNA clearance failure. Blood RNA load represents an important independent predictor of outcome for critically ill COVID-19 patients, with a higher impact on survival compared to respiratory tract SARS-CoV-2 RNA loads.

Pulmonary Thrombosis and Thromboembolism in COVID-19

Hooman D. Poor

Summary: COVID-19 is associated with a significantly increased risk of pulmonary thrombosis, possibly driven by a hypercoagulable state and vascular injury. Some patients with COVID-19 may have hypoxemia that is disproportionate to impairment in lung compliance. Diagnosing PE in patients with COVID-19 can be challenging, leading many hospitals to implement aggressive anticoagulation protocols.

Therapeutic versus prophylactic anticoagulation for patients admitted to hospital with COVID-19 and elevated D-dimer concentration (ACTION): an open-label, multicentre, randomised, controlled trial

Summary: In hospitalized COVID-19 patients with elevated D-dimer concentration, in-hospital therapeutic anticoagulation with rivaroxaban or enoxaparin followed by rivaroxaban to day 30 did not improve clinical outcomes and increased bleeding compared with prophylactic anticoagulation. Therapeutic-dose rivaroxaban and other direct oral anticoagulants should be avoided in these patients without an evidence-based indication for oral anticoagulation.
