Summary: This study examined cardiovascular autonomic injury following acute SCI, finding that two distinct groups emerged at 1 month: autonomically complete SCI individuals with sustained low LF SAP, and autonomically incomplete SCI individuals with increased LF SAP. Autonomically complete injuries did not recover over time, and cardiovascular symptoms worsened, especially in those with autonomically complete lesions, and chronic SCI. Baroreflex function and cardiorespiratory interactions were impaired after SCI, with a higher risk of arrhythmia throughout the acute phase. Acute SCI is associated with severe cardiovascular dysfunction, and LF SAP may provide a valuable assessment of autonomic function 1 month post-injury.

Summary: This study aimed to identify admission risk factors for the development of paroxysmal sympathetic hyperactivity in traumatic brain injury patients. Male sex, higher admission systolic blood pressure, and initial CT evidence of diffuse axonal injury, intraventricular hemorrhage/subarachnoid hemorrhage, complete cisternal effacement, and absence of contusion were independently predictive of paroxysmal sympathetic hyperactivity. Paroxysmal sympathetic hyperactivity cases had worse neurologic outcomes upon hospital discharge despite being matched for injury severity at admission.
