Minor hypertrophic cardiomyopathy genes, major insights into the genetics of cardiomyopathies

Roddy Walsh et al.

Summary: This review explores the complex contribution of genes encoding non-sarcomeric proteins associated with non-syndromic or isolated hypertrophic cardiomyopathy to the genetics of cardiomyopathies. The study highlights the role of various genes in the pathogenesis of cardiomyopathies and the overlapping genetic basis between hypertrophic cardiomyopathy and dilated cardiomyopathy. This overlap extends to rare Mendelian variants with distinct variant classes in several genes associated with both types of cardiomyopathy.

Mavacamten decreases maximal force and Ca2+ sensitivity in the N47K-myosin regulatory light chain mouse model of hypertrophic cardiomyopathy

Peter O. Awinda et al.

Summary: Mavacamten, a pharmaceutical that binds to myosin, reduces isometric tension and Ca2+ sensitivity of contraction in a mouse model of hypertrophic cardiomyopathy expressing the N47K mutation, potentially making it a useful therapy for certain forms of heart disease.

Summary: Aficamten is a novel cardiac myosin inhibitor designed to have predicted human half-life and steady state concentration, with potential clinical applications due to its optimized structure-activity relationship. In a phase I clinical trial, it demonstrated similar human half-life to predictions and achieved steady state concentration within the desired time frame.

Two Classes of Myosin Inhibitors, Para-nitroblebbistatin and Mavacamten, Stabilize β-Cardiac Myosin in Different Structural and Functional States

Sampath K. Gollapudi et al.

Summary: This study examined the effects of two myosin ATPase inhibitors on myosin structure, finding that they can induce different relaxation states. Mavacamten and para-nitroblebbistatin may stabilize myosin in distinct structural states, which could lead to different functional energy-saving states.

The N terminus of myosin-binding protein C extends toward actin filaments in intact cardiac muscle

Sheema Rahmanseresht et al.

Summary: MyBP-C is a rod-like protein located within the sarcomere that interacts with actin and myosin. Super-resolution fluorescence microscopy shows that the N-terminal of MyBP-C is biased towards actin filaments in both active and relaxed muscle, potentially tuning cardiac contractility by capturing molecular binding partners.

Truncated titin proteins in dilated cardiomyopathy

Quentin McAfee et al.

Summary: Truncated titin proteins were quantitatively detected in human DCM hearts with TTNtvs, while full-length titin was reduced in TTNtvs(+) DCM hearts. Disease severity did not correlate with TTNtv location, and transcriptomic profiling showed few differences between TTNtv(+) and TTNtv(-) DCM hearts. Contractility in isolated human adult cardiomyocytes from TTNtv(+) DCM hearts did not show defects compared to TTNtv(-) hearts, supporting a combined dominant-negative and haploinsufficiency mechanism in disease.

Cardiac myosin-binding protein C interaction with actin is inhibited by compounds identified in a high-throughput fluorescence lifetime screen

Thomas A. Bunch et al.

Summary: This study identified small-molecule inhibitors that affect the binding between cMyBP-C and actin, providing potential targets for heart failure drugs that mimic phosphorylation or disrupt interactions with actin/myosin. The fluorescence lifetime assay allowed for the detection of pharmacologically active compounds that interfere with cMyBP-C-actin binding. This validated high-throughput screen focused on cMyBP-C, a regulator of cardiac muscle contractility and a key factor in heart failure, for the first time.
