Assessment of hepatic pyruvate carboxylase activity using hyperpolarized [1-13C]-l-lactate

Summary: This study evaluates the use of hyperpolarized [1-C-13]-l-lactate to detect hepatic pyruvate carboxylase activity in vivo under different nutritional states. The results show that [1-C-13]aspartate can serve as a sensitive biomarker of pyruvate carboxylase flux and can be monitored in vivo at 3 Tesla. The ratio of [1-C-13]aspartate to [C-13]HCO3- reflects the activities of pyruvate carboxylase and pyruvate dehydrogenase enzymes.

Summary: The in vivo assessment of tissue metabolism is a novel strategy for evaluating oncologic disease, particularly hepatocellular carcinoma (HCC). The study found that metabolic alterations, especially in glucose metabolism and lactate production, impact survival differences in HCC tumor tissue. In vivo imaging techniques revealed distinct tumor phenotypes corresponding to differences in glycolytic metabolism in HCC tumor tissue.

Advances in systemic therapies for HCC, including immunotherapies and targeted therapies, are expected to revolutionize the management of this disease.

In Vivo Magnetic Resonance Spectroscopy of Hyperpolarized [1-13C]Pyruvate and Proton Density Fat Fraction in a Guinea Pig Model of Non-Alcoholic Fatty Liver Disease Development After Life-Long Western Diet Consumption

Summary: This study used hyperpolarized magnetic resonance spectroscopy and proton density fat fraction magnetic resonance imaging techniques to investigate the effects of chronic exposure to a Western diet in an animal model resulting in non-alcoholic fatty liver disease. The results showed that lifelong consumption of the Western diet led to increased liver PDFF and triglyceride content, along with decreased lactate TTP and increased LDH activity in the liver. These findings suggest that fat accumulation in WD-exposed livers is associated with a shift from oxidative metabolism to anaerobic glycolytic metabolism.

Summary: The study developed a novel post-processing pipeline for measuring pyruvate-to-lactate conversion rates in patients with liver tumors by integrating tensor denoising and B-+(1) correction. This technical approach not only recovered metabolite signals in voxels that did not receive the prescribed flip angle, but also increased the extent and accuracy of k(PL) estimations throughout the tumor and adjacent regions.
