The impact of community nurse-led interventions on the need for hospital use among older adults: An integrative review

Summary: An integrative review of 9 studies showed that specialist high-intensity, team-based hospital-at-home services and individual case management services incorporating self-help education for chronic disease management can reduce the need for hospital use among older adults. Despite being costly, these services can lead to significant efficiency savings in the long-term.
