Summary: The study aimed to assess the prevalence, characteristics, and severity of unintended medication discrepancies and errors at admission and discharge, as well as identify factors associated with the risk of these discrepancies. It found that the number of treatments and certain medical conditions were associated with an increased risk of medication discrepancies, and that the best type of source for detecting these discrepancies was the general practitioner or nurse.
