1,25-dihydroxyvitamin D as Predictor of Renal Worsening Function in Chronic Kidney Disease. Results From the PASCaL-1,25D Study

Summary: This study aimed to investigate the ability of 1,25(OH)(2)D to predict PTH increase and WRF in CKD stage 3-4. The results showed that lower levels of 1,25(OH)(2)D independently predicted the absolute increase in PTH, while higher levels of 1,25(OH)(2)D were associated with a reduced risk of WRF. The 1,25(OH)(2)D/PTH (1-84) ratio may serve as a composite biomarker of tubular reserve/endocrine response in CKD-MBD.

Differential diagnosis of vitamin D-related hypercalcemia using serum vitamin D metabolite profiling

Summary: In this study, using precision vitamin D metabolite profiling, researchers identified unique vitamin D metabolism profiles in some patients with idiopathic hypercalcemia (IH) and Williams-Beuren syndrome (WBS), suggesting hypersensitive expression of vitamin D-dependent genes as a potential mechanism of hypercalcemia in these patients. Normalization of serum calcium and vitamin D metabolites in an IH patient at follow-up indicates that symptomatic IH may be related to vitamin D nutritional status. This work highlights the importance of serum vitamin D metabolite profiling in differential diagnosis of vitamin D-related hypercalcemia and guiding appropriate treatment.

Mild Idiopathic Infantile Hypercalcemia-Part 2: A Longitudinal Observational Study

Summary: Idiopathic infantile hypercalcemia is a rare disorder with variable clinical features and the response to dietary calcium and vitamin D restriction in mild cases is not able to consistently normalize elevated 1,25(OH)2D concentrations or prevent worsening of renal calcification. Therapeutic options should focus on the defect in vitamin D metabolism.
