Is the Obesity Paradox in Cardiac Surgery Really a Myth? Effect of Body Mass Index on Early and Late Clinical Outcomes

Summary: This study investigated the obesity paradox and the impact of body mass index (BMI) on clinical outcomes after cardiac surgery. The findings showed that obese patients had better survival rates post-surgery compared to overweight and normal weight patients, but BMI was not an independent predictor of in-hospital and one-year mortality when adjusting for other risk factors.

However, there were no significant differences found in chest tube drainage. Further prospective randomized studies are needed to confirm these findings.

Blood Conservation and Hemostasis in Cardiac Surgery: A Survey of Practice Variation and Adoption of Evidence-Based Guidelines

Summary: The survey revealed widespread adoption of several best practices in cardiac surgery, such as tolerance of lower hemoglobin transfusion triggers and use of antifibrinolytics, but gaps remain in preoperative anemia management and the use of transfusion algorithms supplemented with POC hemostasis testing. Practices with a multidisciplinary patient blood management team were the most successful in meeting all criteria, indicating the importance of a coordinated approach in improving blood conservation and hemostasis practices.
