Phase II Randomized Trial of Transoral Surgery and Low-Dose Intensity Modulated Radiation Therapy in Resectable p16+Locally Advanced Oropharynx Cancer: An ECOG-ACRIN Cancer Research Group Trial (E3311)

Robert L. Ferris et al.

Summary: This study aimed to investigate the efficacy and safety of primary transoral surgery (TOS) and reduced postoperative radiation therapy (RT) in intermediate-risk patients with human papillomavirus-associated oropharynx cancer (HPV+ OPC). The results showed that TOS and reduced RT achieved favorable oncologic outcomes and functional outcomes in intermediate-risk HPV+ OPC patients.

Randomized Trial of Radiotherapy Versus Transoral Robotic Surgery for Oropharyngeal Squamous Cell Carcinoma: Long-Term Results of the ORATOR Trial

Anthony C. Nichols et al.

Summary: The study found that for early-stage OPSCC, radiotherapy performed better in terms of long-term swallowing quality of life compared to transoral robotic surgery plus neck dissection, although this difference decreases over time. Some differences in QOL concerns gradually resolved in the long term, but it is worth noting that TORS patients started using more nutritional supplements at 3 years, and dry mouth scores increased over time in RT patients.

Reduced-Dose Radiation Therapy for HPV-Associated Oropharyngeal Carcinoma (NRG Oncology HN002)

Sue S. Yom et al.

Summary: Reducing radiation treatment dose in patients with good-risk human papillomavirus-associated oropharyngeal squamous cell carcinoma (OPSCC) could potentially improve the quality of life. The study showed that reducing radiation dose could achieve disease control and survival rates equivalent to standard chemoradiation.

Oncologic and survival outcomes for resectable locally-advanced HPV-related oropharyngeal cancer treated with transoral robotic surgery

Christina M. Yver et al.

Summary: The study showed that up-front trans-oral robotic surgery (TORS) for locally-advanced HPV-related oropharyngeal cancer provides favorable survival outcomes, with no significant difference in 5-year overall survival and recurrence-free survival compared to early-stage tumors. Despite locally-advanced cases being more likely to have higher stage disease, there was no significant difference in survival outcomes between the two groups.
