Short-course radiotherapy followed by chemotherapy before total mesorectal excision (TME) versus preoperative chemoradiotherapy, TME, and optional adjuvant chemotherapy in locally advanced rectal cancer (RAPIDO): a randomised, open-label, phase 3 trial

Renu R. Bahadoer et al.

Summary: The RAPIDO trial aimed to reduce systemic relapses in locally advanced rectal cancer through short-course radiotherapy, chemotherapy, and delayed surgery. Results showed a decreased probability of disease-related treatment failure in the experimental group compared to standard care, with diarrhea being the most common adverse event and neurological toxicity being prominent in the standard care group during adjuvant chemotherapy.

Primary Tumor Resection Plus Chemotherapy Versus Chemotherapy Alone for Colorectal Cancer Patients With Asymptomatic, Synchronous Unresectable Metastases (JCOG1007; iPACS): A Randomized Clinical Trial

Yukihide Kanemitsu et al.

Summary: This study investigated whether primary tumor resection (PTR) before chemotherapy improves survival in patients with colorectal cancer (CRC) with asymptomatic primary tumor and synchronous unresectable metastases. The results showed that PTR followed by chemotherapy did not provide a survival benefit over chemotherapy alone for CRC patients with asymptomatic primary tumors and synchronous unresectable metastases.

Neoadjuvant chemotherapy with FOLFIRINOX and preoperative chemoradiotherapy for patients with locally advanced rectal cancer (UNICANCER-PRODIGE 23) : a multicentre, randomised, open-label, phase 3 trial

Thierry Conroy et al.

Summary: In patients with cT3 or cT4 M0 rectal cancer, intensifying chemotherapy with FOLFIRINOX before preoperative chemoradiotherapy significantly improved disease-free survival outcomes and reduced neurotoxicity. Thus, the PRODIGE 23 results have the potential to impact clinical practice.

Primary Tumor Resection for Rectal Cancer With Unresectable Liver Metastases: A Chance to Cut Is a Chance for Improved Survival

Jia-nan Chen et al.

Summary: About half of rectal cancer patients develop liver metastasis, with a large proportion being unresectable. Surgical resection of the primary tumor offers the greatest chance of survival for patients with unresectable liver metastases. Prospective studies are needed to confirm these findings.

Capecitabine Versus Active Monitoring in Stable or Responding Metastatic Colorectal Cancer After 16 Weeks of First-Line Therapy: Results of the Randomized FOCUS4-N Trial

Richard A. Adams et al.

Summary: Despite strong evidence of disease control with maintenance therapy, overall survival (OS) remains unaffected. The FOCUS4-N trial demonstrated significant efficacy in progression-free survival (PFS) with maintenance capecitabine therapy, but no significant improvement in OS. Toxicity between the capecitabine and active monitoring groups was comparable, with expected side effects such as fatigue, diarrhea, and hand-foot syndrome.
