Evolution of the stroke paradigm: A review of delayed recanalization

Summary: Although many patients are not eligible for reperfusion within 24 hours according to current guidelines, case studies show favorable outcomes for delayed recanalization beyond 24 hours after symptom onset. Modern imaging techniques aid in identifying penumbra and improving understanding of reperfusion risks and complications. There is an urgent need to further comprehend the pathophysiologic mechanisms and optimal conditions for delayed recanalization after 24 hours of ischemia.

Good Clinical Outcome Decreases With Number of Retrieval Attempts in Stroke Thrombectomy Beyond the First-Pass Effect

Summary: Endovascular therapy for acute anterior circulation stroke is more likely to lead to good clinical outcomes when successful reperfusion is achieved within the first few retrieval attempts, with the odds decreasing with each additional attempt. It is concluded that at least 3 retrieval attempts should be made in endovascular therapy for anterior circulation strokes.

Lack of Reperfusion Rather Than Number of Passes Defines Futility in Stroke Thrombectomy A Matched Case-Control Study

Summary: This study found that patients who achieved successful reperfusion after multiple passes during mechanical thrombectomy had better functional outcomes and higher rates of independence at 90 days compared to those with failed reperfusion. Using arbitrary uncontrolled thresholds to predict futile recanalization may lead to premature termination of procedures, highlighting the importance of individualized patient care in these cases.
