Clinical Importance of Left Atrial Infiltration in Cardiac Transthyretin Amyloidosis

Francesco Bandera et al.

Summary: This study characterized left atrial pathology in explanted hearts with transthyretin amyloid cardiomyopathy (ATTR-CM); assessed left atrial mechanics using echocardiographic speckle tracking in a large cohort of ATTR-CM patients; and studied the association with mortality. The findings revealed extensive atrial infiltration of ATTR amyloid, loss of atrial function, and increased stiffness, which were strong independent predictors of mortality. A distinctive phenotype named atrial electromechanical dissociation (AEMD) was identified in patients in sinus rhythm with poor prognosis.

Anticoagulation with warfarin compared to novel oral anticoagulants for atrial fibrillation in adults with transthyretin cardiac amyloidosis: comparison of thromboembolic events and major bleeding

Lindsey R. Mitrani et al.

Summary: The study found that patients with ATTR-CA and AF are at increased risk for stroke compared to those without AF. There was no significant difference in primary outcomes between AF patients treated with warfarin and NOACs, with similar rates of thrombotic events and major bleeds observed.

Arterial thrombo-embolic events in cardiac amyloidosis: a look beyond atrial fibrillation

Francesco Cappelli et al.

Summary: In a large multicentric cohort of CA patients, the prevalence and incidence rate of AEs was high, with a significant proportion of events occurring despite anticoagulation therapy or in patients in sinus rhythm. A higher CHA2DS2-VASc score may identify patients at risk of AEs, even among those in sinus rhythm.

Diagnosis and treatment of cardiac amyloidosis: a position statement of the ESC Working Group on Myocardial and Pericardial Diseases

Pablo Garcia-Pavia et al.

Summary: Cardiac amyloidosis is a serious infiltrative disease caused by the deposition of amyloid fibrils in the heart, which can be due to genetic variants or acquired conditions. With advances in imaging techniques, non-invasive diagnosis is now possible, showing that cardiac amyloidosis is more common than previously thought. The Working Group on Myocardial and Pericardial Disease has proposed definitions and diagnostic algorithms for cardiac amyloidosis.

Left atrial structure and function of the amyloidogenic V122I transthyretin variant in elderly African Americans

Summary: This study showed that carriers of the V122I TTR variant in African-Americans have significant left atrial enlargement and dysfunction compared to non-carriers, suggesting that abnormalities in left atrial function may be early markers of subclinical disease in this population.

Summary: In patients with cardiac amyloidosis, there are differences in clinical characteristics and outcomes between those receiving DOAC and VKA anticoagulation therapy, with DOACs appearing to be at least as effective and safe as VKAs.

Diagnosis and treatment of cardiac amyloidosis: a position statement of the ESC Working Group on Myocardial and Pericardial Diseases

Pablo Garcia-Pavia et al.

Summary: Cardiac amyloidosis is a serious and progressive infiltrative disease caused by amyloid fibril deposition in the heart. Advances in imaging techniques and diagnostic algorithms have made it possible to diagnose the condition non-invasively. Monitoring and treating cardiac amyloidosis are important in bridging the gap between research advances and clinical practice.
