Possible reasons for the regrettable results of patency of an inside stent in endoscopic transpapillary biliary stenting

Masaki Kuwatani et al.

Summary: Endoscopic biliary decompression is a minimally invasive procedure for cholestasis that plays an important role in patient management. Stent patency is crucial for patient prognosis and treatment planning. Factors such as food impaction, biofilm formation, and sludge formation can lead to stent occlusion, with higher patency expected in suprapapillary EBS compared to transpapillary EBS.

Diagnostic power of DNA methylation markers suggestive of cholangiocarcinoma in ERCP-based brush cytology

Varayu Prachayakul et al.

Summary: This study demonstrated that the methylation index of HOXA1 and NEUROG1 genes could improve the sensitivity and accuracy for the diagnosis of CCA. Combining methylation markers with brush cytology and CA19-9 further increased diagnostic power. DNA methylation shows promising potential for the diagnosis of CCA in patients with biliary strictures, warranting further validation studies for clinical application.

Summary: This study compared the efficacy of combined ERCP and EUS-BD with bilateral PTBD in patients with MHBO, and found that the former had significantly lower RBO rates at 3 and 6 months. Additionally, the former also had fewer reinterventions and longer time to RBO occurrence.

A novel technique for mapping biopsy of bile duct cancer

Hirokazu Okada et al.

Summary: This study investigated the usefulness of a novel technique for mapping biopsies of perihilar cholangiocarcinoma (PHCC) using a biliary stent delivery system. The results showed adequate sampling rate, high rate of negative margins in resected specimens, and no occurrence of post-endoscopic retrograde cholangiopancreatography pancreatitis.

Summary: This study found that pCLE had high sensitivity in detecting cholangiocarcinoma in patients with PSC and dominant biliary strictures, but technical limitations may affect evaluation in certain areas of the biliary tract. Further research is needed to clarify the role of pCLE in the diagnostic algorithm for biliary strictures associated with PSC.

Factors affecting the diagnostic yield of endoscopic transpapillary forceps biopsy in patients with malignant biliary strictures

Min Jae Yang et al.

Summary: This study evaluated the factors associated with a positive cancer diagnosis from transpapillary biliary forceps biopsy (TBFB) and found that bile duct cancer, intraductal growing type, and five or more tissue samples were significant predictors of positive TBFB results. Increasing the number of tissue samples by five or more led to higher sensitivity in patients with bile duct cancer with the periductal infiltrating type.

Summary: The use of inside stents in preoperative biliary drainage may reduce the need for re-intervention and prolong the time to re-intervention in patients with malignant perihilar biliary strictures, potentially providing significant benefits for these patients.

The role of stent placement above the papilla (inside-stent) as a bridging therapy for perihilar biliary malignancy: an initial experience

Summary: This study compared the safety and efficacy of IS placement and ENBD as bridging therapy for potentially resectable PHBM patients. The results showed that IS placement had lower catheter dislocation rates and longer time to recurrent biliary obstruction compared to ENBD, suggesting IS placement as a possible alternative with similar postoperative complication rates for patients undergoing resection.
