Glycaemic management in diabetes: old and new approaches

Antonio Ceriello et al.

Summary: The modern approach to glycemic control emphasizes not only achieving and maintaining optimal HbA(1c) concentrations, but also reducing postprandial hyperglycemia, glycemic variability, and extending time in near-normoglycemia. Hypoglycemia should be avoided, and if it occurs, should be reverted to normoglycemia. Utilizing modern technology and new drug therapies can help to shift the focus of glycemia management from HbA(1c) towards a more holistic approach considering all aspects of diabetes pathophysiology.

Relationship Between Time in Range, Glycemic Variability, HbA1c, and Complications in Adults With Type 1 Diabetes Mellitus

Anass El Malahi et al.

Summary: This study investigated the association between glycemic variability and time in range with chronic and acute diabetes complications. The results showed that lower time in range was associated with the presence of microvascular complications and hospitalization for hypoglycemia or ketoacidosis. However, glycemic variability was not associated with macrovascular complications.

Time in Range in Relation to All-Cause and Cardiovascular Mortality in Patients With Type 2 Diabetes: A Prospective Cohort Study

Jingyi Lu et al.

Summary: The study found that lower TIR was associated with increased risk of all-cause and CVD mortality among patients with type 2 diabetes. This suggests the validity of TIR as a surrogate marker for long-term adverse clinical outcomes.

Glucose variability and diabetes complications: Risk factor or biomarker? Can we disentangle the Gordian Knot''?

Louis Monnier et al.

Summary: Variability in glucose homeostasis plays a significant role in diabetes complications, especially in relation to hypoglycemic events. It is important to avoid excessive glucose fluctuations within a day and to understand that variability in glucose homeostasis is not only associated with cardiovascular events but also a causative factor for hypoglycemia.

Associations of hypoglycemia, glycemic variability and risk of cardiac arrhythmias in insulin-treated patients with type 2 diabetes: a prospective, observational study

Summary: This study found that cardiac arrhythmias were common in insulin-treated patients with type 2 diabetes, and were associated with glycemic variability rather than hypoglycemia.

Glycemic variability and cardiovascular disease in patients with type 2 diabetes

Marcela Martinez et al.

Summary: Glycated hemoglobin is crucial for evaluating long-term glycemic control in diabetes patients, but it does not account for blood glucose fluctuations. Recent clinical data suggest that glycemic variability is associated with increased risks of complications in diabetes patients, independently of glycated hemoglobin levels.

Quality of Glycemic Control: Assessment Using Relationships Between Metrics for Safety and Efficacy

David Rodbard

Summary: This review suggests that only two metrics are sufficient for monitoring patients or comparing different forms of management interventions: one for efficacy evaluation and one for safety evaluation. By analyzing the relationships between safety and efficacy metrics, sensitivity can be improved and the need for arbitrary scoring systems can be avoided, offering the advantage of simplicity in both concept and practice.

Association of HbA1c With All-cause Mortality Across Varying Degrees of Glycemic Variability in Type 2 Diabetes

Jingyi Lu et al.

Summary: The relationship between HbA1c and all-cause mortality among patients with type 2 diabetes varies depending on the degree of glycemic variability. In patients with high glycemic variability, HbA1c may not be a robust marker of mortality, suggesting the need for new metrics for better diabetes management in these individuals.
