Antimicrobial resistance in paediatric Streptococcus pneumoniae isolates amid global implementation of pneumococcal conjugate vaccines: a systematic review and meta-regression analysis

Summary: This study reviewed antimicrobial susceptibility studies of pediatric pneumococcal isolates from 2000 to 2020, finding significant reductions in non-susceptibility of pneumococci to penicillin, sulfamethoxazole-trimethoprim, and third-generation cephalosporins after implementation of PCV products. The findings suggest that PCV implementation has led to decreased resistance of pneumococci to first-line antibiotic treatments for pneumonia.

Effect of Amoxicillin Dose and Treatment Duration on the Need for Antibiotic Re-treatment in Children With Community-Acquired Pneumonia The CAP-IT Randomized Clinical Trial

Summary: In a clinical trial involving 814 children with community-acquired pneumonia, it was found that lower-dose oral amoxicillin (35-50 mg/kg/day) was noninferior to higher dose (70-90 mg/kg/day), and a 3-day regimen was noninferior to 7 days in terms of the need for antibiotic re-treatment. However, various factors such as disease severity, treatment setting, prior antibiotics received, and acceptability of the noninferiority margin need to be considered when interpreting the findings.

Antibiotic therapy in children with community-acquired pneumonia

Summary: The review of PubMed on antibiotic treatment of community-acquired pneumonia (CAP) in children for the years 2011-2020 found that five-day courses were equally effective as longer courses. No new antibiotics were launched for children's CAP during the last 10 years, and stewardship of antibiotics needs lessening of exposure to antibiotics and shortening the lengths of antibiotic courses.

Antibiotics for lower respiratory tract infection in children presenting in primary care in England (ARTIC PC): a double-blind, randomised, placebo-controlled trial

Summary: The study found that amoxicillin is unlikely to be clinically effective in reducing symptom duration in children with uncomplicated lower respiratory tract infections, overall or in key clinical subgroups where antibiotics are commonly prescribed. Unless pneumonia is suspected, clinicians should not prescribe antibiotics for most children presenting with chest infections.
