Natural history of polycystic ovary syndrome: A systematic review of cardiometabolic outcomes from longitudinal cohort studies

Summary: This study reveals that the regulation of uterine androgen receptor (AR) levels is crucial for normal pregnancy, with abnormal AR regulation potentially contributing to pregnancy-related complications in women with polycystic ovary syndrome (PCOS). Exposure to DHT and INS during pregnancy leads to elevated uterine AR levels, impacting endometrial receptivity and decidualization gene expression. Treatment with the anti-androgen flutamide can prevent these defects, improve mitochondrial homeostasis, and enhance pregnancy outcomes.

Decidualized endometrial stromal cells present with altered androgen response in PCOS

Summary: The study revealed distinct transcriptome profiles between decidualized and non-decidualized endometrial stromal cells from PCOS and Ctrl samples, with PCOS-specific genes involved in mitochondrial function and progesterone signaling. Addition of DHT resulted in altered responses related to lysophosphatidic acid, inflammation, and androgen signaling pathways, highlighting potential defects in decidualized PCOS cells and providing insights into the mechanisms of endometrial dysfunction in PCOS.

Endometrial function in women with polycystic ovary syndrome: a comprehensive review

Summary: Studies have shown differences in endometrial function in women with PCOS compared to healthy controls, with clinical characteristics of the syndrome contributing to dysregulation. Current interventions with the highest scientific evidence for clinical benefit in improving endometrial dysfunction in PCOS women include lifestyle modifications, metformin, and bariatric surgery. Further large-scale randomized controlled clinical trials are needed to address possibilities for reversing endometrial dysfunction in this population.
