Fibrinolysis Shutdown and Thrombosis in a COVID-19 ICU

Summary: The COVID-19 pandemic has led to serious complications including systemic inflammation, organ dysfunction, and thromboembolic disease. Within the institution, a study on critically ill patients found a high prevalence of fibrinolysis shutdown among those who experienced thrombotic events. This suggests the potential benefit of using viscoelastic testing to identify patients who may benefit from fibrinolytic therapy.

Summary: Elevated fibrinogen levels and hypercoagulable blood clot characteristics were identified in critically ill COVID-19 patients through ROTEM testing. However, COVID-19 patients with thrombotic complications had lower, yet still hypercoagulable, ROTEM measurements of fibrinogen compared to those without. Further research is needed to validate these findings and investigate the mechanisms driving these relationships for better diagnostic and treatment approaches.
