The effectiveness of chemonucleolysis with condoliase for treatment of painful lumbar disc herniation

Eijiro Okada et al.

Summary: Chemonucleolysis with condoliase is a safe and effective treatment for painful LDH, with 85.4% of patients showing improvement without severe adverse events. Factors associated with clinical effectiveness include young age, no history of epidural or nerve root block, and injection into the central portion of the intervertebral disc. Optimal outcomes are achieved when condoliase is injected into the center of the intervertebral disc.

Kasra Amirdelfan et al.

Summary: The study evaluated the safety and efficacy of intradiscal injection of STRO-3+ adult allogeneic mesenchymal precursor cells (MPCs) combined with hyaluronic acid (HA) for chronic low back pain (CLBP) associated with degenerative disc disease (DDD). Results showed that MPC therapy was superior to controls in improving VAS and ODI and was well tolerated without clinical symptoms of immune reaction. The therapeutic effects were sustained up to 36 months.

Intradiscal glucocorticoids injection in chronic low back pain with active discopathy: A randomized controlled study

Isabelle Tavares et al.

Summary: The study showed that intradiscal injection of prednisolone acetate can significantly reduce pain intensity at 1 month for patients with low back pain and active discopathy, but there was no significant difference at 3 and 6 months. Additionally, the GC group showed significantly better daily activities in the Dallas Pain Questionnaire at 1 and 3 months compared to the L group.

Micro-mechanical damage of needle puncture on bovine annulus fibrosus fibrils studied using polarization-resolved Second Harmonic Generation (P-SHG) microscopy

J-Y Wang et al.

Summary: Needle injection is widely used in spinal therapeutic or diagnostic processes, but it has been suspected to cause mild disc degeneration leading to long-term back pain. This study found that needle punctures cause local microscopic damage to collagen and cell nuclei, resulting in progressive damage. The size of the needle used did not significantly affect the damage, with more damage observed using larger needles.

THE EFFECT OF HYALURONIC ACID ON NUCLEUS PULPOSUS EXTRACELLULAR MATRIX PRODUCTION THROUGH HYPOXIA- INDUCIBLE FACTOR-1A TRANSCRIPTIONAL ACTIVATION OF CD44 UNDER HYPOXIA

F. Zhang et al.

Summary: This study revealed that under hypoxic conditions, hyaluronic acid (HA) enhances the expression of extracellular matrix genes and proteins in nucleus pulposus cells (NPCs), as well as increases CD44 expression. Hypoxia inducible factor-1 alpha (HIF-1α) plays a key role in the transcriptional activation of CD44, further promoting matrix synthesis in NPCs in the presence of HA.
