Socioeconomic Inequalities in Home-Care Use Across Regional Long-term Care Systems in Europe

Summary: This study examines whether socioeconomic inequalities in home-care use among disabled older adults are related to the contextual characteristics of long-term care systems. The findings suggest that in regions with more de-familized LTC systems, there is lower socioeconomic status inequality in the use of mixed-care. Poorer individuals are more likely to combine informal and formal home-care use in regions with more LTC beds, potentially mitigating health inequalities in later life.

Ethical challenges in home-based care include autonomy, balancing ethical principles, intensity of care decisions, priority settings, truth-telling, and professional role balancing. These challenges are similar to those in institutional healthcare and social care settings, with some significant differences highlighted.
