Vascular and metabolic effects of SGLT2i and GLP-1 in heart failure patients

Michele Correale et al.

Summary: Alterations in endothelial function, inflammatory activation, and nitric oxide-cGMP pathway play a role in the pathophysiology of heart failure. Sodium-glucose co-transporter 2 inhibitors (SGLT2i) and glucagon-like peptide-1 (GLP-1) receptor agonists (RAs) have emerged as potential treatments for heart failure, but their protective mechanisms are not fully understood.

Pathogenesis of chronic heart failure: cardiovascular aging, risk factors, comorbidities, and disease modifiers

Filippos Triposkiadis et al.

Summary: Chronic heart failure (HF) is rare in the young and common in the elderly. The causes of HF in the young are usually specific, such as adult congenital heart disease, cardiomyopathies, myocarditis, or cardiotoxicity. On the other hand, the mechanisms underlying HF development in the elderly have not been fully understood. It is proposed that in most elderly patients, HF is the consequence of cardiovascular aging accelerated by specific risk factors, such as hypertension, obesity, type 2 diabetes mellitus (T2DM), coronary artery disease (CAD), and valvular heart disease (VHD), which affect both the heart and the vasculature.

Outcomes and mortality associated with atrial arrhythmias among patients hospitalized with COVID-19: A systematic review and meta-analysis

Lukasz Szarpak et al.

Summary: This study conducted a meta-analysis to investigate the impact of AF/AFL on mortality and complications in COVID-19 patients. The results revealed that AF/AFL is associated with increased in-hospital mortality and worse outcomes. Patients with AF/AFL also have a higher risk of ICU admission and complications such as bleeding, acute kidney injury, and heart failure.

Thrombotic Complications in Patients with COVID-19: Pathophysiological Mechanisms, Diagnosis, and Treatment

Aleksandra Gasecka et al.

Summary: Emerging evidence suggests that severe COVID-19 cases are associated with an increased risk of thromboembolism, with one-third of hospitalized patients developing macrovascular complications. Diagnosis of COVID-19-associated coagulopathy can be challenging due to overlapping features with pulmonary embolism, but early recognition of coagulation abnormalities is crucial for guiding treatment and improving patient outcomes. Recommendations for anticoagulant use in hospitalized COVID-19 patients vary and should be made on a case-by-case basis.

Summary: Obesity is a significant risk factor for arrhythmic cardiovascular death. A review investigates the influence of epicardial adipose tissue (EAT) on arrhythmogenesis, highlighting its impact on atrial and ventricular conduction delay, structural cross talk with the heart, and secretion of substances that influence cardiomyocyte electrophysiology. The interaction between EAT and cardiomyocytes facilitates arrhythmias via activation delay, conduction slowing, and increased risk of block.

Summary: This study examined the association between epicardial adipose tissue (EAT) measured on chest CT and pneumonia severity and adverse outcomes in 109 COVID-19 patients. It was found that EAT volume and attenuation were independent predictors of clinical deterioration or death, along with total pneumonia burden, chronic lung disease, and history of heart failure. The results suggest that EAT measures could be used in clinical risk stratification for COVID-19 patients.

This suggests that genes in PCAT may play a role in CAD development and progression.
