Spanish COPD Guidelines (GesEPOC) 2021 Update Diagnosis and Treatment of COPD Exacerbation Syndrome

Juan Jose Soler-Cataluna et al.

Summary: This article provides detailed recommendations on the diagnosis and treatment of COPD exacerbation syndrome (CES). The guidelines propose a syndromic approach, a new severity classification, and the recognition of different treatable traits (TT), aiming to personalize medicine. The diagnostic process includes four stages, and the treatment varies depending on severity and specific traits.

Do Redox Balance and Inflammatory Events Take Place in Mild Bronchiectasis? A Hint to Clinical Implications

Liyun Qin et al.

Summary: In mild bronchiectasis patients, systemic inflammation and redox imbalance may correlate with clinical parameters, with important inflammatory and oxidative stress events taking place in plasma. These findings have therapeutic implications for management of bronchiectasis patients.

Systemic Profiles of microRNAs, Redox Balance, and Inflammation in Lung Cancer Patients: Influence of COPD

Liyun Qin et al.

Summary: MicroRNAs and redox imbalance play a role in lung tumorigenesis in COPD patients. This study found differential expression profiles of microRNAs in LC patients, as well as increased plasma oxidative stress levels and decreased GSH in LC patients with COPD. These systemic alterations may contribute to the pathogenesis of tumorigenesis in these patients.

Phenotypic Clustering in Non-Cystic Fibrosis Bronchiectasis Patients: The Role of Eosinophils in Disease Severity

Xuejie Wang et al.

Summary: This study investigated whether high blood eosinophil counts could define a better phenotype in bronchiectasis patients and identified two distinct clinical phenotypes based on blood eosinophil counts. Patients above the threshold exhibited significantly better clinical outcomes, lung function, and nutritional status while showing lower systemic inflammation levels. These results contribute to better characterizing bronchiectasis patients into phenotypic profiles with clinical implications.

Differences in Nutritional Status and Inflammatory Biomarkers between Female and Male Patients with Bronchiectasis: A Large-Cohort Study

Xuejie Wang et al.

Summary: Gender-related differences were found in non-CF bronchiectasis patients, with females showing less severe disease, lower degree of airway obstruction, better nutritional parameters, and lower levels of inflammatory biomarkers compared to males. Nutritional parameters and inflammatory biomarkers may be reliable indicators of gender-related differences in this patient population.

Defining Modern Pulmonary Rehabilitation An Official American Thoracic Society Workshop Report

Anne E. Holland et al.

Summary: Pulmonary rehabilitation is effective for chronic lung disease but underused globally. New models aim to improve access. Workshop identified essential components and requirements for implementation. Delphi process determined key components and need for further research on patient characteristics. Personalized assessment and quality assurance are crucial for success. Future success will depend on delivery of key components and achieving expected outcomes.
