Summary: Diabetic neuropathy is a common chronic and progressive complication of diabetes mellitus that strongly affects patients' quality of life. The etiopathogenesis of diabetic neuropathy is multifactorial, with a role played by genetic components, gene polymorphisms, and epigenomic mechanisms. These factors are significant for the diagnosis, therapy, and prevention of diabetic neuropathy.

Summary: Lipid dysmetabolism is a key feature of diabetes mellitus, including impaired cholesterol metabolism, increased lipid uptake and synthesis, increased fatty acid oxidation, lipid droplet accumulation, and imbalance in biologically active sphingolipids. Controlling renal lipid dysmetabolism is an important therapeutic goal for DKD, as it is associated with increased reactive oxygen species production, oxidative stress, inflammation, and cell death.

Pre-emptive pharmacological inhibition of fatty acid-binding protein 4 attenuates kidney fibrosis by reprogramming tubular lipid metabolism

Summary: The study found that in the process of kidney fibrosis, the expression of FABP4 increased, accompanied by the accumulation of lipid droplets in TECs. Treatment with FABP4 inhibitor BMS309403 alleviated lipid deposition in TECs and reduced interstitial fibrotic responses. Additionally, BMS309403 enhanced fatty acid oxidation in TECs and reduced cell lipotoxicity in fibrotic kidney.
