Reduced discrimination based on qAF measures was associated with mild disease, more advanced disease with dark flecks, or older age due to age-related increase in qAF and a ceiling effect in patients.

Summary: This study aimed to establish mutation-specific age-dependent UWF-FAF trajectories in a large cohort of STGD1 patients and develop a clinical method for variant classification based on TLS. Different ABCA4 mutation types were found to have unique age-dependent trajectories of TLS, with mutations classified into severe, intermediate, and mild impairment groups based on TLS and age of symptom onset. Further molecular studies are needed to validate this new clinical criterion.

The c.769-784C>T variant is found to be a major modifier of the p.(Gly1961Glu) allele, contributing to the rarity of affected homozygotes in the population.
