Improved prediction of mortality by combinations of inflammatory markers and standard clinical scores in patients with acute-on-chronic liver failure and acute decompensation

Henning Gronbaek et al.

Summary: This study demonstrates that specific inflammatory markers in combination with clinical scores can better predict survival in ACLF patients than the original scoring systems. The findings suggest a key role of macrophage activation and inflammation in the development and progression of ACLF.

On-treatment serum albumin level can guide long-term treatment in patients with cirrhosis and uncomplicated ascites

Paolo Caraceni et al.

Summary: Baseline serum albumin concentration closely correlates with 18-month mortality in untreated patients; on-treatment serum albumin levels are strongly associated with outcomes, especially at 1 month after treatment. Long-term albumin administration improves patient survival even if the serum albumin level does not normalize.

Hepatorenal syndrome in children: a review

Summary: Hepatorenal syndrome is a kind of pre-renal failure due to intense reduction of kidney perfusion induced by severe hepatic injury. The pathogenesis is multifactorial and treatment includes fluid expansion therapy, discontinuation of nephrotoxic drugs, and pharmacological treatment with vasoconstrictors when necessary. Further studies are needed to investigate early biomarkers and alternative treatments for HRS.

Characterization of Blood Immune Cells in Patients With Decompensated Cirrhosis Including ACLF

Emmanuel Weiss et al.

Summary: Patients with ACLF exhibit dysregulation of blood immune cells, including increased neutrophils and M0-like monocytes, as well as depletion of various lymphocyte subsets. These immune cell alterations, along with defective neutrophil superoxide anion production, may contribute to immunosuppression in ACLF, highlighting potential targets for future therapies.

Impact of the Gut Microbiome on the Progression of Hepatitis B Virus Related Acute-on-Chronic Liver Failure

Xuebing Yao et al.

Summary: This study found that the diversity and abundance of the gut microbiome significantly decreased as hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF) progressed, with potentially pathogenic bacteria showing increased abundance in HBV-ACLF patients compared to healthy controls. The abundance of Bacteroidetes was negatively correlated with serum alpha fetoprotein, while the abundance of Veilonella was positively correlated with serum total bilirubin levels. These findings suggest that the gut microbiota plays an important role in the development of HBV-ACLF.
