A 78-Year-Old Man with a Pulmonary Embolism Who Developed Skin Necrosis 7 Days After Treatment with the Direct Oral Anticoagulant Factor Xa Inhibitor Apixaban

Summary: This case report describes a 78-year-old man who developed severe skin necrosis on both forearms and abdominal wall 7 days after starting apixaban for pulmonary embolism. Skin biopsy was not performed due to bleeding risk, but clinical findings supported the diagnosis of skin necrosis caused by apixaban. The patient's condition gradually improved with wound care, and he was discharged safely with subcutaneous low-molecular-weight heparin therapy. This report highlights the potential skin toxicity associated with apixaban and emphasizes the need for clinicians to be aware of such adverse effects with the increasing use of newer DOACs.
